Step One: Getting Started

The first step in our process is getting started. Submit the form to the right and someone from our staff will be in touch with you to schedule an appointment.


Step Two: Enrollment

All new patients must get signed up in our patient portal system. This portal system is your personalized link to paperwork, medical history, treatment plans, and billing information. All initial paperwork including registration forms and financial disclosures needs a signed consent for treatment to begin.


Step Three: Appointment

Our registration staff will contact you for scheduling your first appointment. This first appointment can either be scheduled for thirty minutes, one hour, or one hour and thirty minutes. During this visit, a full assessment is performed to determine if our practice meets your needs and if so, what treatment plan is recommended. Upon completion of the appointment, your physical exam, intake forms, and treatment plans are loaded into your personalized patient portal account for easy access.


Step Four: Visit our Patient Portal

While you wait for our team to contact you to schedule your first appointment, visit and join our Patient Portal to start the paperwork essential for visiting a physician at The Well.
